1 full cycle for women under 43. Women 40-42 must not have had previous IVF. Same-sex/single women require 6 self-funded AI cycles (3 if >36) plus 6 NHS-funded IUI cycles before IVF. Unexplained infertility requires 2 years expectant management or 12 AI cycles. Donor eggs permitted for 40-42 if AFC<=4, AMH<=5.4, or FSH>8.9. [ICSI: Indications include severe semen deficits, azoospermia, or previous IVF failure. 1 full cycle for women under 43. Women 40-42 must not have had previous IVF.
